Follow these instructions to download and complete a membership application:

  1. Click on the button to download the form.
     
  2. Select the option to Open the form rather than Save it.
    The form will open using Microsoft Word.
     
  3. Type in responses to each unprotected field.
    Use the Tab key to move from field to field.
     
  4. Print the completed form.
     
  5. Close the form window.
    Respond No when asked if you want to save the information.
     
  6. Sign the form.
     
  7. Mail the completed and signed form with payment to the address indicated.

    Drop it off a the FRGC pro shop.
